If this will be your first visit with us please download and review all three documents prior to your first meeting with a physician. This will shorten your wait in the office and improve the exchange of information.

We ask that you review our Privacy Policy. This complies with Federal policy mandated by the 1996 Health Insurance Portability and Accountability Act (HIPAA). This policy describes your rights with regard to access and protection of your private health information.
As part of our Privacy Policy, we ask that you provide phone numbers that you would like us to use when calling for appointments, test results or scheduling surgery.
Please complete both pages of our medical history form. If you need additional space for details of medical problems, operations or lists of medications attach separate pages as required.
