Do hemorrhoids have a relationship with sitting?

Hemorrhoids — especially external hemorrhoids — can cause a great deal of pain and discomfort, making sitting an extremely uncomfortable task. However, because sitting can put increased pressure around the anal area, sitting for long periods can also cause new hemorrhoids to develop. 

The pressure of sitting is particularly magnified when on the toilet. Even if you’re not straining during a normal bowel movement, sitting on the toilet for long stretches can increase pressure on your anus and pelvic floor. For instance, reading or browsing your phone for a long time on the toilet can cause strain, even if you’ve already evacuated your bowels.

How to sit with hemorrhoids?

Whether you have external or internal hemorrhoids, the best way to sit with hemorrhoids is to use a soft memory foam cushion on your seat and elevate your legs slightly in the air. If your sitz bath and hemorrhoid cream don’t seem to be working, the culprit might be your sitting position. Let’s take a deeper look. 

1. Sit on a soft surface 

Sitting on a hard surface puts increased pressure on your gluteal muscles and can cause the area around your anal canal to stretch, forcing blood vessels to swell out further. So if you’re prone to hemorrhoids, avoid sitting on a hard surface for an extended period. 

Another word of caution — try to avoid sitting on a donut pillow for pain relief when sitting with hemorrhoids. While many people mistakenly believe they are a solution for hemorrhoids, that might not be true.

Since these circular pillows do not have a center, they actually increase pressure on your rectum and pelvic floor and can make hemorrhoid swelling worse. This option may seem appealing since it lifts your anorectal area off of the hard surface you’re sitting on, but it actually adds pressure to your anorectal region and is therefore not recommended. 

2. Sit on a memory foam seat cushion

If you’re looking for a soft seat to support you while you sit, a memory foam seat cushion can be a great help. It can provide relief from pressure that can otherwise exacerbate hemorrhoids and support your lower body to relieve pressure on your pelvic floor and rectum. 

These cushions are made from a material that naturally conforms to your body, so using them is like sitting on a pillow made especially for your body shape. 

Sitting on memory foam pillows can also improve lower body circulation and reduce the risk of constipation, alongside providing support and relief from hemorrhoid pain. 

3. Position on the toilet 

The best position for passing stool is to squat over the toilet. Squatting with your knees against your abdomen creates a better alignment for the release of a bowel movement and is also believed to reduce the risk of hemorrhoid development. 

If squatting is uncomfortable for you, sit on the toilet as you would normally but try raising your feet using a step stool. By bringing your knees above your hips, you alter the angle of your rectum, giving stool a more direct route out of the body. This reduces the chances of over-straining and pressure in your anal area. 

And as mentioned above, avoid sitting on the toilet for long periods if you’re constipated. The wide opening of a toilet seat promotes anorectal stress and can worsen your hemorrhoids or trigger another flare-up. So, instead of sitting for very long, get up and move around to help stimulate your bowels. 

You also want to cut the time you spend on the toilet by performing regular exercise, eating a lot of fiber, and increasing your water intake. This will ensure regular, easy bowel movements, reducing the amount of time you feel the need to sit on the toilet.

4. Keep your legs elevated 

Elevating your legs off the floor reduces the pressure that sitting puts on your spine, rectum, and hips. 

So, try keeping a foam wedge of a rolled-up towel under your thighs to alleviate pressure from your rectal region. You can also use a footrest to get your feet off the floor if you’re sitting for longer periods.

Is it better to walk or sit with hemorrhoids?

Generally, walking around is better than sitting if you have hemorrhoids. This is because a lack of physical activity and sitting for extended periods can make you more prone to hemorrhoidal flare-ups.

In fact, a study from South Korea surveyed around 17000 hemorrhoid sufferers and found that not walking enough was one of the key contributing factors to their condition. 

Of course, this doesn’t mean that you should never sit when you have hemorrhoids. But don’t assume you shouldn’t be walking if you have the condition — it can actually help relieve your pain and itching. 

What is the best sleeping position with hemorrhoids?

The best way to sleep with hemorrhoids is to make sure you don’t put pressure on your anal area. There are different ways you can achieve this.

For example, you could try sleeping on your side (fetal position) or stomach. If that’s too uncomfortable, you can try placing a soft pillow under your legs to elevate them and reduce the pressure on your anal region. You also want to use a soft mattress — ideally one that’s made of memory foam — to stay comfortable.

Hemorrhoids are a common medical condition that affects millions of people worldwide. Despite this, they often go unnoticed due to the stigma and embarrassment surrounding them. 

A few weeks ago, a patient showed up at our out-patient clinic and described having symptoms of hemorrhoids. He mentioned he could feel “something coming out of the anus” and had blood in stools. 

Unfortunately, he was too embarrassed to let us examine his anal area — which is needed to confirm the diagnosis — and left the clinic without treatment.

This made us realize that while the physical symptoms of hemorrhoids are widely discussed, the emotional and psychological challenges people with this condition face are often brushed under the rug. 

So, in this article, we will explore the psychological impact of hemorrhoids, including strategies for overcoming the taboo surrounding this condition.

What are hemorrhoids?

Before delving into the psychological aspects of the condition, it’s first important to understand what hemorrhoids are. 

Also known as piles, hemorrhoids are swollen veins in the rectum or anus. They can be external, developing under the skin around the anus, or internal, occurring inside the anal canal. 

Since many people consider hemorrhoids too embarrassing to share with their healthcare providers — only 4% seek help — the actual number of people with hemorrhoids is impossible to determine. 

However, research shows that at least 50% of Americans will have experienced hemorrhoidal symptoms by the time they reach the age of 50.

What leads to hemorrhoids?

Hemorrhoids are caused by excessive pressure on the blood vessels around the anus. The most common cause of hemorrhoids is constipation, where people strain to move their bowels.

Overcoming the taboo surrounding hemorrhoids is essential for raising awareness, promoting timely treatment, and supporting those affected. Here are five important strategies to break the silence and reduce the stigma associated with the condition.

Topical treatments have been shown to be effective in several studies. For instance, a 2015 study showed that PP110 Wipes, Preparation-H®, and PP110 Gel reduce swelling by 24.3%, 11%, and 9.2%, respectively. 

It also revealed that Preparation-H® reduced pain and discomfort levels by 10%, while PP110 Gel led to a 2% improvement. PP110 Wipes, however, caused a 3% increase in discomfort and pain symptoms. 

So, if you’re looking for ways to reduce pain and other hemorrhoidal symptoms, you should try Preparation-H® and PP110 Gel. 

b. Pain relievers 

Here are some OTC painkillers you can take:

• Ibuprofen (Advil and Motrin)
• Aspirin 
• Acetaminophen (Tylenol and Feverall). 

However, you should avoid using non-steroid anti-inflammatory drugs (NSAIDs) like ibuprofen because they can cause rectal bleeding to become worse. 

Aside from that, you can also use ointments with lidocaine and tribenoside — such as Lidoderm, LidaMantle, and Regenecare — to relieve pain symptoms. These have been shown to be “excellent” at reducing pain, bleeding, itching, and burning, especially when combined.  

For instance, in a clinical study, patients experienced a 93% reduction in bleeding, 72% in anal itching, and 56% in pain when using a combination of lidocaine and tribenoside creams. 

c. Stool softeners 

Straining and constipation are the primary contributors to hemorrhoid development, with both increasing damage around anal tissues. Drugs with stool-softening agents can reduce these instances by allowing stool to absorb more water and fat, causing it to soften. 

Some stool-softening drugs or laxatives include bisacodyl (Dulcolax) and docusate (Correctol). 

2. Home remedies for pain relief

You can also try several effective home remedies to reduce inflammation and speed up hemorrhoid shrinkage.

a. Take sitz baths 

If you’ve been struggling with pain caused by hemorrhoids, a sitz bath is a time-honored way to relieve it. Here’s what to do:

1. Fill a sitz tub or a rectangular inflatable pool with three inches of lukewarm water. 
2. Sit in the bath for 15 minutes at least three to four times daily. This will help decrease pain, reduce swelling, and relax your muscles. 

You can also use Epsom salts like magnesium sulfate if you want to during your bath. These are thought to relax your muscles, reduce inflammation, and tone down swelling. However, more studies are needed to prove these claims. 

b. Use cold compresses and ice packs

Constant exposure to air or moisture can cause mild to severe itchiness. If you’re struggling with itching, you can use cold compresses and ice packs to numb the area. Make sure to wrap them in a thin cloth so you don’t shock your muscles and cause burns. 

You may have to apply cold compresses several times throughout the day for long-term relief. 

c. Wear loose cotton clothing 

Clothing made from acrylic and nylon doesn’t allow moisture to wick away, which increases itchiness and discomfort and is a good way to irritate hemorrhoids. So, try to wear loose cotton clothing — preferably lawn or Egyptian cotton — to reduce pain. 

d. Reduce the itching with witch hazel 

Witch hazel is an astringent that soothes and temporarily relieves the discomfort, burning, and itching caused by hemorrhoids. It works by removing moisture and tightening the skin. You can apply it externally to get relief from external hemorrhoids. 

Some witch hazel astringents include: 

• Preparation-H® wipes and pads
• Tucks® wipes
• Boiron Hamamelis Virginiana 30c. 

e. Apply coconut oil to the area 

Coconut oil is anti-inflammatory, which means it can reduce swelling and inflammation. It also has pain-relieving or analgesic properties, helping reduce pain and discomfort caused by hemorrhoids. 

So, if the area around your hemorrhoids feels inflamed or painful, you can apply it topically to reduce these symptoms. But if that doesn’t work, you could mix coconut oil with witch hazel and apply the mixture to your hemorrhoids using a cotton ball. It’ll soothe the area and relieve pain.

You can also shape coconut oil into bullets using a silicone mold or cut them into appropriate widths. You can freeze these suppositories for later use. 

How to prevent hemorrhoids?

So, those were some ways to treat hemorrhoids and make them shrink fast. However, prevention is the best medicine when it comes to this condition. But which changes should you make to your lifestyle to avoid these painful lumps from popping up again and again? Let’s take a look at the best: 

1. Increase your fiber intake

Straining can damage the muscles around the anus, causing hemorrhoids. Fiber supplementation in the form of psyllium (Metamucil) or other soluble fiber options can eliminate straining by enhancing water absorption, reducing gut transit time, and improving gut motility. 

In fact, clinical studies show fiber can reduce the risk of bleeding, itching, prolapse, and pain by 47% to 50%. Six-week and three-month follow-ups also showed consistent results over time, and one study also suggested a significant decrease in recurrence. 

So you should eat at least 28 grams of fiber per 2,000 calories consumed, according to the Dietary Guidelines for Americans, 2020–2025. 

2. Stay hydrated 

An inadequate fluid intake can cause your body to think you don’t have enough water, which means it takes out as much water from your stools as possible. This leads to hard, lumpy, and dry stools that are hard to pass during a bowel movement. 

Plus, in older adults, liquid deprivation from 500 to 2500 ml per day leads to significant dehydration and constipation, which can cause hemorrhoids. To avoid that from happening, drink at least six to eight glasses of water daily to keep your stools soft. 

3. Don’t strain 

Straining or sitting on the toilet for too long waiting to “go” puts pressure on the blood vessels in and around your anus. This can lead to tissue damage and cause blood to pool in the area, increasing your risk for hemorrhoids. 

To make sure that doesn’t happen, avoid sitting on the throne for longer than five minutes if nothing is happening. You can always come back. 

4. Avoid medication that causes constipation 

If you’re struggling with hemorrhoids, you should avoid constipation at all costs. This means eating as much fiber as possible and drinking enough water. However, it also means avoiding medications that cause constipation. 

Here are some medications you should be aware cause constipation:

• Iron supplements
• Nausea medications like Zofran and Phenergan
• Narcotic pain medications like ConZip, Fentanyl, OxyContin, and Dilaudid
• Antacids with calcium, such as Maalox
• Antidepressants like Zoloft and Norpramin 
• Blood pressure medicines like Norvasc, Cardizem, and Catapres
• NSAIDS like Advil, Aleve, and Motrin. 

Also, keep in mind that some vitamins may contain iron. You should avoid these if you’re struggling with recurring hemorrhoids. 

When should you see your doctor?

While hemorrhoids can be annoying, paying a visit to your doctor isn’t a must. But there are times when you have to go, such as: 

1. Persistent or severe symptoms

Mild hemorrhoids can cause itching, pain, and discomfort, but they don’t leave you unable to sit upright or move without crying. If that happens, you most likely have a severe case and must schedule a visit with a colon and rectal surgeon who treats hemorrhoids. 

Similarly, if you’re experiencing problems despite trying OTC ointments and at-home treatments, you should go for a visit. In fact, you should consult your doctor the moment you feel a little pain.  

2. Recurring hemorrhoids

If your hemorrhoids keep coming back, something might be amiss with your diet or toilet habits. You might not be able to pinpoint these issues yourself, so going to a doctor is a must. They’ll help you understand which dietary and toilet habits you need to change. 

Your doctor will also prescribe medications that can stop hemorrhoids from occurring so frequently. 

3. Rectal bleeding or dark stools

What is Crohn’s disease?

Crohn’s disease is a chronic inflammatory bowel disease (IBD) that can affect any part of the gastrointestinal tract, from the mouth to the anus (unlike ulcerative colitis). 

It’s characterized by inflammation of the gastrointestinal tract lining, which can lead to a wide range of symptoms, including abdominal pain, diarrhea, weight loss, and fatigue.

Can Crohn’s disease cause hemorrhoids?

No, Crohn’s disease doesn’t directly cause hemorrhoids. However, the inflammation and irritation associated with Crohn’s disease can lead to anal fistulas and abscesses. 

These, in turn, can make bowel movements extremely painful and uncomfortable. As people become more hesitant to pass stool, they may become constipated. And constipation is a well-known risk factor for hemorrhoids.

Why does constipation lead to hemorrhoids?

Constipation leads to hemorrhoids because it puts extra pressure on the veins in your anus and rectum. This pressure can cause the veins to swell and eventually bulge, leading to hemorrhoids.

The best way to reduce your risk of hemorrhoids is to take measures to reduce the inflammation in your gastrointestinal tract. This is especially true if your CD is affecting the area around your anus (peri-anal area), making bowel movements painful.

So how would you know if CD is affecting your peri-anal area? Well, if you develop an anal abscess as a result of your CD, you’ll experience things like:

• Dull discomfort around the anal area
• Excessive itching around the anal area
• A red mass under the skin of the peri-anal area
• Fever with chills

In case you develop an anal fistula as a result of your CD, you’ll experience:

• A foul-smelling, pus-containing liquid draining from the skin around your anal area
• Pain while passing stool

If you experience any of these symptoms, it’s important to seek medical attention as soon as possible. That’s because unlike regular CD, which may be treated with medication, peri-anal CD usually requires surgery.

Even if you don’t have peri-anal CD, though, it’s still important to take measures to reduce the inflammation in your gastrointestinal tract. This can be done through a combination of medication and lifestyle changes (such as eating a healthy diet).

Also, keep in mind that you don’t necessarily have to have a CD in order to develop an anal abscess or peri-anal fistula. Both of these conditions can affect anyone, regardless of whether they have a CD or not.

Can you check hemorrhoids without a colonoscopy?

Yes, hemorrhoids can be checked without a colonoscopy. This is in contrast to CD, which often requires a colonoscopy for diagnosis.

This means that if your healthcare provider suspects that your hemorrhoids are due to underlying CD, they might perform a colonoscopy. This is especially true if you have red flag symptoms that are not common in simple hemorrhoids, such as:

• Weight loss
• Long-term fever

On the other hand, if you have grade 4 hemorrhoids, you’ll probably need to undergo a surgical procedure called hemorrhoidectomy.

For your reference, hemorrhoids are graded as follows:

• Grade I: hemorrhoids that bleed but do not prolapse (protrude from the anus)
• Grade II: hemorrhoids that prolapse but spontaneously reduce (go back inside the anus on their own)

That’s because, in this blog post, we’ll answer all your questions about getting surgery for a perianal fistula. But first, let’s briefly review what it really means.

What is a perianal fistula?

A perianal fistula is a small tunnel that forms between the anus and the skin near it. The most common cause of a perianal fistula is an infection of the anal gland. But there are also other causes, such as:

• Inflammatory bowel disease, especially Crohn’s disease
• Acute infections of the gut, such as diverticulitis
• Radiation
• Colorectal cancer
• Foreign body

Symptoms can range from anal itching, burning, and pain to drainage of pus or blood. You might even experience fever or chills.

Types of fistulas

There are many types of perianal fistula, but they’re broadly divided into high fistulas and low fistulas. The internal opening of a high fistula is located above something called the dentate line. For low fistulas, this opening is located below the dentate line.

The dentate line is a line of tissue that separates the anal canal from the rectum. It’s important to know where your fistula is located, as it will determine the kind of surgery you’ll need.

How is a perianal fistula treated?

A perianal fistula is treated with a combination of drugs and surgery.

Anal abscess treatment

A perianal fistula begins as a rectal abscess.  This is a collection of pus in the lining of the anal canal or rectum.  Usually, patients present to the doctor with acute pain and fever. This is caused by the perianal abscess, which is very common in people with a fistula. In this case, it’s important first to treat the abscess and then the fistula.

Types of anal abscesses (source)

A perianal abscess is treated by a procedure called incision and drainage, where a doctor makes a cut in the abscess and drains out the pus. This allows the infection to heal faster.

After the procedure, you may be recommended stool softeners, pain-killers, Sitz baths, and even antibiotics in some cases. 

Once the inflammation around your anal area has completely healed, the doctor will then move on to performing a definitive surgical procedure to close the fistula.  It is important to note that not all abscesses become chronic fistulas.  Some heal and do not return.  This can occur in 50% of patients.

Surgical treatment for perianal fistula

The current standard surgical treatment for perianal fistulas is called a fistulotomy. This involves making an incision along the length of your fistula and then cutting it open, which results in the complete removal of the fistula.

However, this procedure is not without risks. Complications such as incontinence, bleeding, infection, and poor wound healing may occur, especially if the fistula involves a large portion of the anal sphincter muscle. 

In this case, your doctor may place a seton, which is a thread-like material, around the fistula and leave it in place for several weeks. This is done to slowly allow the body to heal and reduce the risk of incontinence.  Also, if the fistula involves a large amount of anal sphincter, the surgeon will not want to cut all of that muscle for fear that the patient will become incontinent.  This would mean losing a lesser or greater amount of control of the anal sphincter and thus, losing control of sool, liquid or gas.

Another commonly performed procedure is called an endorectal advancement flap (ERAF). This is where the surgeon uses the inner lining of your rectum to cover the internal opening of the fistula. The goal is to prevent communication between the internal and external openings and promote fistula healing. This avoids injury to the sphincter complex and has low incontinence risks.

LIFT — ligation of intersphincteric fistula tract — is a relatively new procedure to treat fistulas, but there are not a lot of studies to determine how effective it is. This involves dividing the fistula and tying off the ends, allowing the tissues to heal with almost no risk of incontinence.

Crohn’s fistula treatment

In case you have a perianal fistula due to Crohn’s disease, your healthcare provider will ensure your Crohn’s disease is under control before going for surgery. 

Unfortunately, people with Crohn’s tend to get recurrent fistulas, even after treatment. This may prompt your doctor to perform a procedure called panproctocolectomy, where the entire colon and rectum are removed.

Sometimes, your healthcare provider may inject something called ‘fibrin glue’ into your fistula. This promotes the formation of a clot inside the fistula, closing it off mechanically. Fibrin glue injection is thought to be a simple, effective, and safe procedure for perianal fistulas due to Crohn’s disease. Success rates can be highly variable.
